CDC Monthly Reporting Template. On the 10th business day of each month, we will need to submit our monthly reporting to the CDC. This is a link to the form where we collect everyone's input and send to CDC. This next report is due February 14th and therefore we ask you submit any input by noon EST, Thursday, February 10th.

**HRSA Reporting Template.** Each organization has it's own tab. Please use this form to fill out your organizations (including any of your subcontracts) metrics monthly. The next reporting period is Feb 1 - Feb 28. Please submit this form by noon EDT, Wednesday 3/3/2022. Note: There is new line item on row 109 for you to report your % spend of your total grant (not distributed funds) as of September 30th (column H). For example, if your total grant is $100,000 and you have received $40,000 and have spent $25,000 as of September 30, you would enter 25%. ******

If your communities have begun activities, please make sure they are filling out your community sub awardee worksheet. This information can inform what you report in the HRSA reporting template. Below are the worksheets for each organization:

A. Surveys for the Community outreach worker : This should be completed by every staff member who is engaged in outreach (Spanish version Community outreach worker).

B. Brief survey while doing phone, door to door, event, or digital outreach**:**

1. Brief Outreach form (under 65): This form is the shorter survey for those under 65. (Spanish brief Outreach Form (Under 65))
2. B**rief Outreach form (over 65)**: This form is the shorter survey for older adults over 65. (Spanish brief Outreach form (Over 65))
3. Longer form 1:1 Outreach form: (Under 65) includes vaccine equity and other questions some partners wanted): this form is to be used if requested by your organization to get additional information on vaccine equity and other questions some partners requested for those under 65.
